The church is known for enshrining the Black Nazarene, a dark statue of Jesus Christ, which many claim to have miraculous powers. The church hosts a weekly novena every Friday, and is visited by thousands of devotees every day.

Every January 9, millions of devotees celebrate the translacion, or transfer of the Black Nazarene statue at the church by way of a daylong procession. It is stated that once you have physical contact with the statue, all of your prayers become true.
